Hi, I would like to know if over-torquing a screw retained PFM could cause implant failure in 3 weeks?

Pt came for delivery of Screw retained crown tooth #12 (UCLA Gold Abutment from Straumann) for a 3.3 BL SLActive Straumann implant 3 weeks ago. (The Impression visit/removal of healing cap that was done 4 weeks prior was uneventful.). I wasn’t able to deliver crown on the first visit because the screw would not engage with implant. I sent it back to the lab who said the interface/apical orifice was slightly compressed, not allowing the screw to seat correctly, and he corrected it by pushing thru the orifice with a backend of a bur. (The lab told me that this can occur when the crown is placed without the screw inside.). The following day, I delivered the crown under local anesthesia, verified no occlusal interference with excursive movements, created light occlusion on centric groove, and torqued to 25Ncm. Pt presented 3 weeks later for his cleaning visit and complained that he had pain 5/10 scale since crown delivery which had now dissipated. He also noted that he could hear creaking on the tooth when he ate or pressed with his finger on the lingual cusp. Upon evaluation, I noted that crown rocked with finger pressure on B/L cusp. I thought crown was loose so I loosened screw, fully seared crown and tightened. The crown still rocked. I then removed the crown town completely and viewed implant/implant crown internally: besides gingival inflammation, all appeared normal. I seated crown, hand tightened screw, torqued to 25 Ncm, waited a few seconds and tightened again to 25 Ncm, which caused his crown to rotate 10-15 degrees clockwise. Pt indicated he felt pain. I loosened the screw which allowed his crown to shift back into the original position. I rechecked his occlusion and no rocking was evident; I found he occluded on #12 buccal cusp only when he overextended his mandible forward and left. I reduced B cusp 3 mm but he still could occlude; I didn’t reduce further thinking that he would not do such an exaggerated movement. I dismissed him and told pt I would talk with OS. Pt returned next day as I wanted to verify whether his condition indicated implant failure. I lightly anesthetize pt with removed crown and placed healing cap. On hand and wrench tightening, pt had no pain but sensation of pressure. Implant did not spin and had a firm stop. On loosening healing cap, pt felt pain. I tightened healing cap again and referred to OS for evaluation. On the emergency OS visit, OS told me he suspects implant failure; he will definitively test on next visit by reverse torquing the implant under local anesthesia and if there’s no resistance, the it’ll be definitively an implant failure.

So, my patient feels that the overtorquing the crown had caused the implant failure but I have not been able to find any literature showing that. I would like to know if restorative procedures can cause implant failures in 3 weeks? I thought occlusal discrepencies would cause crown to loosen and over torquing would cause screw loosening. Pt does have wear on his teeth but wears NTI nightly.
