I have this question.

Female Patent , 56 years of age, with good oral hygiene and good home dental care. Tooth 26 ( Maxillary left 1st molar) had unsatisfactory MO amalgam with lingual extension, with distal craze stained line.

we changed the amalgam and add the distal craze line and removed some caries that was leaking in the existing amalgam from the distal wall on March 2018.

Patient came in November 2018 for some shooting pain coming from teeth 23,24 and we noted that the buccal abrasion was the reason , once we covered it that shooting pain subsided.

However, we replaced , in July 2019, the Mo portion of tooth 26 and replaced the DO part of tooth 25 at no charge to address the discomfort that patient experiences when she flosses( as she say and I noted that when I remove the floss and as it clicks through the contact area , it gives the patient shock feeling.

Today, after replacing the restoration I still see the same discomfort and I am seeking your advise as I am not sure what is causing this shock feeling discomfort. I personally suspecting a micro-crack but I am not sure yet that this is the cause , and I don't recall that I saw a crack line on the pulpal floor.

what do you think the cause of the discomfort and what would be the treatment that would address the issue?
