The chip is on a Feldspathic Veneer. Do you have better success with the Kuraray kit. I do have both HF and KGel Etch as well in my office. Its a tiny chip near the gingival margin.

Hello All
I have a good friend that used to practice dentistry, She had porcelain veneers placed years ago by a Prostho classmate.
#10 has a small chip at the gingival third of the veneer. I apologize that I did not take any photos. It has been a long time since I have posted anything on Spear Talk.
Any advice on repairing this? I am asking with respect to any specific materials and techniques
For anyone asking, the Prostho that did the veneers is in Canada and this mutual friend has 2 small children and I offered to repair this for her.
Any thoughts/advice would be welcomed.

Hello,
My daughter is congenitally missing #7. She is 15 years old and just completed orthodontic treatment. To my knowledge, it is best to wait until complete growth has happened. My periodontist had recommend at least age 18. I am more inclined to wait as I am sure her face/bone/gingiva will continue to grow and change. Please have any ortho or perio or OS Drs give their input.

Thank you for all of your responses. I really appreciate it. Rodney, ortho was finished at 12. In my opinion, there is a general ‘rush’ I feel that parents and kids prefer to straighten the teeth faster than waiting. While I can understand being proactive sometimes I feel that giving the jaws time to develop is also important. This is purely my opinion. I’m just seeking advice from the practioners that have been around longer to see situations like this play out over years. I am now considering attending the Occlusion workshop sometime soon. Would any of you recommend Occlusion workshop first vs. Facially Generated Treatment Planning?
Again, I really appreciate feedback thus far.

Hello Everyone,
I was looking for any help/advice on treatment approaches for a 14 y.o. female with TMJ pain.
This young lady has pain to open wide and jaw makes popping sound. She completed ortho 2 years ago and when I called the orthodontist discuss, she indicated that she does not recommend an occlusal nightguard. Are there any Spear videos on tmj and younger patients? Would imaging be important from the start (MRI)? This young lady is a singer. I have not typically come across a patient this young with tmj issues. When she initially indicated symptoms a few months ago we decided to use Ibuprofen as needed/warm packs/etc as needed. However her symptoms are still present. Would referral to Oral Surgeon be best? I am hesitant to send to Physical Therapy without joint imaging.
Just concerned due to her young age that she is having joint issues so early in life.
Any input is appreciated

That looks great! I am in favor bonding the pontic to the fixed lingual wire as long as she has the space. Those photos are wonderful. I have to see if we have room on the linguals of the canines for the wire to be bonded lingually. I would rather not prep anything if possible. Since she is about to turn 15 we need something that will last until she is a full grown adult.

This photo was taken at the ortho appointment. They had just removed the wire and the Pontic. During the ortho treatment the Pontic has been attached to the arch wire. I am asking for ideas after the braces come off.

Hello All,
I am looking for any recommendations for a fixed interim solution for my daughter. She is about to have the braces removed in a few months.
Congenitally missing #7. She is 15 years old. The long term solution is the dental implant but we cannot place until she is an adult.
I am looking for any ideas for a Maryland Bridge... EMAX pontic, Composite pontic? Design? Any prep? I have often seen these fail.. Likely design failure?
Any input would be great. We are obviously going to have a Hawley with pontic and an Essex retainer with pontic. I really prefer not to have her eat without a tooth in public if possible.
Looking for a conservative fixed solution.

Unfortunately if he is just a “PPO patient” he may not be ready for treatment. All we can do is show him what we see and educate him on why. Often while patients have stressful personal issues such as divorce they can’t think clearly no matter how we say things..
I agree with all of the advice given. At times all we can do is patch the emergency and then encourage a comprehensive approach, which you have done. My guess is that he will probably come back once his life “settles down” and he will appreciate the detail and thorough exam with photos. We do what we can, and they rest is up to the patient.

I am curious if anyone has had any experience or cases where a patient sought out regenerative/stem cell therapy to treat a TMJ condition? I have a young patient (25y.o) with a Piper 4a/4b condition. Left side disc is off and the right side is better but still high risk. This was confirmed with an MRI recently. We recommended a surgery consult with Dr. Piper. Prior to the MRI I referred her to a fellow dentist in my study club and he has offered some secondary options to manage the TMJ condition since she is trying to avoid surgery. She has not committed to any treatment with him as of yet. She is now seeking other options to treat this condition. I made her a flat plane occlusal splint several months ago and she wears that. This collegue that I referred her to works with Dr. McKee and has been amazing trying to help me work thru this with her. I have completed numerous restorations on her over the past 2 years. Unfortunately I took her case when she had several teeth with decay, temp crowns, failing root canals, etc. Her father is a friend of mine.
I have tried so hard to help this patient. She recently asked for her records and now is coming to see me this evening. I am so reluctant to "approve of" or speak on anything with respect to Stem Cell/Regenerative therapy to treat a TMJ condition this severe.
Does anyone know of any stories/studies good or bad with respect to this type of approach to treat a chronic TMJ condition that is currently flared up? She recently had dental work completed 8 weeks ago with me. I saw her once for a follow up to check the restorations and she did not report any jaw pain on that follow up appt. When she called me yesterday and asked if she could come in this evening for me to check something on her cheek. I asked her how she's been feeling with her jaw and she informed that she has her first treatment tomorrow for Stem Cell regeneration??
My questions are: Is it my place to ask who and what this treatment is? Ask if the person providing this treatment has treated cases like hers with success? Did he review the MRI with her?
Since I have informed her that I do not have extensive experience on managing more complex chronic TMJ, and referred her to another person more qualified, am I still obligated to ask these questions? I am now out of my comfort zone.
I appreciate any advice on this from anyone.

Hi Kevin,
I just sat in a 2 day course with Dr.Jim Metz and he recommended the Minolta i300. Says it gives very good data. I think it might cost about $1000.
Gives you AHI, Oxygen saturation, and Heart rate.
He swears by it.

Hello,
Our family has dental insurance thru my husband's company. I have been seeing my kids and husband for years as patients. My hygienist performs the prophys and I do the exams and any necessary treatment. We submit for the procedures to the insurance company and they pay as they would for any other insured patient. Personally, I see nothing wrong with this. We are licensed Dentists treating patients and billing for the services. It should not matter that they are our children or spouse.
I have not run into any problems. I feel the problems would arise if you billed for procedures that you did not complete. That would be fraud.

Hello Everyone,
I was wondering what the opinion or recommendations would be to restore this tooth. This patient is mid 70's on alot of meds. #14 has an existing crown with a large abfraction below the crown margin. Would replacing the actual crown be best or do any of you have success with a buccal restoration to cover the exposed root? There is not decay present on this tooth.
I will attach a photo. His CC is that food becomes trapped. After discussing with him possible causes he admits to using a medium bristle toothbrush. This is evident on multiple teeth. Some have caries associated with them some do not. If any recommendations on restoring the only the facial, please let me know of your technique and material.

Yes, retraction cord was used. The dark line in only on 1 of the veneers and its on the lingual surface. Should I just re-make it? They were delivered over 1year ago. I will post a photo soon.

Hello All,
I was wondering if anyone could give any insight on veneer stain at the gingival margin. I delivered a case last year and 1 of the veneers has a black line on the lingual ging. margin. It is not decayed. Any thoughts? Case looks great. She loves them!! Is this something that needs to be replaced? If anyone has a video in the library to recommend, I'd appreciate the feedback.

Thank you all for the responses. In this particular case it's an Astra implant size 3.6. The implant is very well placed by my periodontist. The lab actually fabricated the Atlantis Zr abutment and I was supposed to deliver it tomorrow. I decided to call the lab yesterday and switch to a titanium abutment. I was not aware of this, but Atlantis will give only a 5 year warranty on the Zr abutment. They will however offer the lifetime warranty with Titanium. I read that they usually fail at the neck connection mostly during insertion.

Hello everyone,
I have a patient of mine that is experiencing alot of TMJ/ear pain for the past 2-3 months. He has seen an multiple specialists and I would like to refer him to a TMJ Pain Clinic if possible. I made him an occlusal splint and he indicated that it helped for awhile but then he wasn't wearing it as office because he was have sinus problems and could not sleep. I saw him a few weeks ago and adjusted the splint. He said it felt better. I referred him to Endo for a consult to rule out any teeth needing RCT. He has multiple crowns without RCT. The Endo that I use is very thorough and he deemed all teeth test wnl. He has had imaging completed. I would like to know if anyone can recommend any TMJ Pain clinics around the Chicagoland area. I am in Oakbrook. He has also been doing physical therapy sessions.
