Dear colleagues, we all know how to deprogram muscles( thanks to Spear center), but may I ask you one question: is it possible to reprogram them to hold the mandible in a forward and down position relative to CR? For example if my patient has severe class II, and refused to do orthognathic surgery, and ( what makes the case even more difficult) his/her back teeth have worn down almost to the gum line, so no space to make restorations in CR without opening VDO, but the opening VDO in class II patients will lead to even more open bite. Some of my colleagues tell that in these cases they create a repositioning splint to move the mandible forward, and doing that they are achieving a coupling of the front teeth, and at the same time space for restoring back teeth. And after some period of time, they built new occlusion in this position, and they design cusp-fossa relationship in that a way in order to achieve so called retrusion control.

I am concerning:

1) Will this position be reliable and repeatable?

2) How can cusp-fossa relationship ( "retrusion control") prevent the mandible from moving back to CR if the chewing circle starts from the opening of the mouth, and after mouth is open, only muscles responsible for the envelope of function?

3) Is it real to reprogramm muscles to create new envelope of function in a more forward position?
